Contact Information (Name, Business Name, Phone Number)

Contact information for the business or end user, including: (1) the name of the contact person (full given name and family name), (2) the business name when the end user is a business (registered legal name where applicable; may be omitted or stated as 'N/A' for individual end users), and (3) one or more contact phone numbers where the business or end user can be reached. Contact person name and at least one phone number are mandatory; business name is required only when the end user is a business entity. Phone numbers should be in international E.164 format where possible.

ID/Passport Copy or Company Registration Certificate (Depending on Number Owner)

Identity documentation depending on whether the phone number is owned by a business or by a person. For numbers OWNED BY A COMPANY: a copy of the company registration certificate is required. For numbers OWNED BY AN INDIVIDUAL: a copy of the individual's passport or government-issued ID is required. The choice depends on the registered owner of the number, not on who is paying for service. Documents must be clear color copies; expired IDs and unsigned/draft certificates are not acceptable.

The address may be located in any country in the world; this requirement does NOT impose a country match with the phone number. Acceptable documents include utility bills (electricity, water, gas, internet, landline telephone), bank statements, tax notices, government correspondence, or insurance documents. The document must clearly show (1) the full name of the individual or registered business name, (2) the complete physical address, and (3) the document issue date. Mobile phone bills, screenshots, and bills older than 3 months are not acceptable.

Worldwide Physical Address

A valid, real-world physical address located anywhere in the world. Must include street name and number, postal code (where applicable), city, region or province (where applicable), and country. The address provided must correspond to a validated physical address (e.g., as enriched via address validation in the Telnyx system). P.O. boxes, virtual office addresses, and mailbox-only services may not be acceptable depending on the order's downstream country requirements; a deliverable physical location is preferred.

The choice depends on whether the end user is an individual (option a) or a business (option b). ID/passports must show full name, photograph, date of birth, document number, and expiration date, and must not be expired. The country of issuance for ID/passports does not need to match the country of the phone number. Business certificates must show legal name, registration number, registration date, and registered address, and must be issued by a registry in the same country as the phone number.

Address Matching the DID Area Code (Geographic Match)

A physical address that matches the geographic area covered by the phone number's area code. The address must include street name and number, postal/ZIP code, city, and country, and the city/region of the address must fall within the geographic area assigned to the DID's area code. Addresses outside the area code's geographic region are not acceptable for this requirement; a different (broader, national-level) address requirement must be selected if the customer cannot provide a matching local address.

A free-text description of how the phone number will be used. The description must be specific enough to identify the calling activity (e.g., inbound customer support, outbound sales, two-factor authentication, IVR, conferencing, voice broadcasting). Generic responses such as 'business use', 'voice service', or 'phone calls' are not acceptable; a specific activity must be named.
