The document authorizes Telnyx to act on the customer's behalf with respect to the phone numbers (typically for porting). The completed document must be returned with all required fields populated (legal name, address, phone numbers, signing date) and the authorized representative's signature applied. Bank statements are an acceptable form of address verification when they show the required name, local service address, issuer, and date within the past 3 months. Leases, rental agreements, or other documents with occupancy or contract terms are not accepted.

A physical address that matches the geographic area covered by the phone number's area code. Addresses outside the area code's geographic region are not acceptable for this requirement; a different (broader, national-level) address requirement must be selected if the customer cannot provide a matching local address.
